Please fill out a form for each person who is attending *any* troop events this year, whether they are youth Trailmen or adults. If you would prefer to fill out a paper version, you may pick up a copy at the next troop meeting or print one for yourself from this Google Drive Folder.

**FYI:** That link is also where you will find a copy of the ‘High Adventure Form’, which requires a doctor visit and is only needed if planning to attend troop events that exceed 72 hours in duration, or include high altitude or high-exertion activities (e.g. Summer Backpacking trip).
